Three-dimensional conformal radiotherapy for locally advanced cervical cancer: a retrospective analysis of clinical and dosimetric predictors for survival.
The purpose of this study was to retrospectively analyse the clinical and dosimetric predictors of survival in patients with locally advanced cervical cancer treated with definitive three-dimensional conformal radiotherapy (3DCRT). From January 1994 to October 1997, 78 patients with International Federation of Gynecology and Obstetrics stage IIB (n = 60) and IIIA (n = 18) cervical cancer treated with 3DCRT were included in this retrospective study. Initial and follow-up staging studies included a computed tomography scan of the pelvis and abdomen and a gynecologic examination. The median follow-up period was 18 months (range, 6 to 58 months). The median age of the patients was 53 years (range, 29 to 75 years). All patients were treated with 3DCRT in a radiotherapy department in accordance with the International Commission on Radiation Units and Measurements report 62 and recommendations of the German working group for radiation oncology. The median total dose was 59.4 Gy (range, 45 to 68.4 Gy) and the median fraction size was 1.8 Gy (range, 1.6 to 2.2 Gy). The overall survival rate was estimated using the Kaplan-Meier method and the Cox proportional hazard model was used for univariate and multivariate analyses. The median survival time was 42 months (range, 6 to 56 months). Only FIGO stage (p = 0.02) and the median age (p = 0.02) were prognostic factors for survival on univariate analysis. On multivariate analysis, FIGO stage (p = 0.007) and the median age (p = 0.04) were prognostic factors for survival. The survival rate of patients with locally advanced cervical cancer treated with 3DCRT may be improved with the adoption of individualized treatment concepts. We found that patients with FIGO stage IIB had a better survival rate than those with IIIA. Further improvements of survival rates may require dose escalation and further investigations of new regimens, such as chemoradiotherapy.Q:
